Written State Complaint Exit Surveys

This document, developed in partnership with members of CADRE’s Intensive TA Written State Complaint Workgroups and Senior Consultant, Dr. Tim Hedeen, includes survey items that may be incorporated into a state’s written state complaint exit survey.  Exit surveys can help states obtain feedback from participants in the written state complaint process and ensure that services are continually improved and refined.
